Insurance and Billing

At Pro Spine & Pain, we prioritize our relationship with you, not your insurance company. Your insurance coverage is a contract between you and your carrier. It’s important to note that not all medical services at our facility are covered by every insurance plan. We strongly encourage you to become familiar with your insurance benefits and any limitations. If you have any questions regarding your coverage, feel free to reach out to your insurance provider directly.

Co-Payments, Co-Insurance, & Deductibles

All co-pays, co-insurance and deductibles are due at the time of your appointment.

Let’s break it down:

  • A co-payment is a fixed fee that you must pay before your insurance company will cover any expenses. The amount of the co-payment depends on your insurance plan.
  • Co-insurance is a percentage of the total cost that you must pay after your deductible has been met.
  • A deductible is the amount you must pay out of pocket before your insurance will start covering expenses. The patient must pay the deductible before the benefits of the insurance policy can apply.

Pro Spine & Pain is committed to providing high quality medical services to our patients. We offer our patients several payment options to help meet their financial obligations to us.

  • Secure electronic funds transfer from checking or savings account

Additional Insurance Information

  • New patients must complete patient information forms online or at our office. Before your appointment, we will ask you to verify your insurance information.
  • Please remember to bring all of your insurance identification cards with you on the day of your appointment. We will make photocopies to keep on file.
  • Providing accurate information at the time of your visit will help us process your claims promptly. If you have multiple insurance companies, please let us know which one is the primary carrier. If there are any changes to your health coverage, please inform us in advance. Your cooperation in keeping your information up to date is greatly appreciated.
  • Please be aware that there may be additional costs not covered by your insurance. Depending on your policy, you may be responsible for co-payments and other deductible costs for our services.
